I hereby consent to and authorize the makeup artist, A'Kylah Hemphill, at The Pampered Way LLC to perform a makeup application.

I give permission to the makeup artist named above to apply makeup to my skin. I have made her aware of any allergies or skin sensitivity I may have.

I give my makeup artist permission to take photos of the makeup she has applied on me to use before and after comparison, educational purposes, and documentation.

Although it is impossible to list every potential risk and complicaton, I have been informed of possible risks and complications. I have also, to the best of my knowledge, given an accurate account of my medical history, including all known allergies or prescription drugs or products I am currently ingesting or using topically.

I affirm that I am 18 years of age or older, and competent to sign this release on my own behalf.

I have read and fully understand this agreement and all information detailed above. I understand this service and accept the risks. All of my questions have been answered to my satisfaction and I consent to the terms of this agreement. I do not hold the makeup artist A'Kylah Hemphill or The Pampered Way LLC responsible for any of my conditions that were present, but not disclosed at the time of this makeup service, which may be affected on day of service rendered.
